Alpha Academy is dedicated to ensuring the health and safety of every student while on campus. To support students who require medication during school hours, we provide the Physician’s School Medication Form, which allows authorized school personnel to administer prescribed medication according to a physician’s instructions. This form must be completed by both the student’s licensed healthcare provider and the parent or legal guardian. It includes detailed information about the medication, dosage, administration schedule, and any special storage requirements to ensure proper care is delivered throughout the school day.

Parents and guardians are encouraged to review the requirements carefully before submitting the form. All medications must be in properly labeled pharmacy containers, and students are not permitted to transport medication to or from school. Non-medical personnel may administer medications daily, and the school nurse is available one day each week. This form helps streamline communication between families, medical professionals, and the school to maintain a safe learning environment. Completed forms are valid for one year and must be renewed annually or as directed by a physician.
